Please allow a 1 week response time when sending in intake forms.

The intake packet is one of the first steps in starting services at Ruby Beach Behavioral Pediatrics, LLC. Please follow the link below to access and fill out the intake packet and send in a copy of your insurance card and your child's psychological evaluation. Insurance will ask for these items when authorization for services is requested.

Other relevant documentation that is helpful, but not required, to submit with the intake packet includes:​

• Speech Evaluations

• Occupational Therapy Evaluations

• Individualized Education Plans

• Any other assessments pertaining to developmental goals
